Scheduling
Our office staff will make every effort to accommodate your schedule when making an appointment.
Please give us 48 hours notice when changing appointments. This will enable us to schedule another patient in need of our services.
Please assist us at your initial visit to our office by providing the following:
• Referral slip from your dentist
• Recent x-rays (please bring or request that they be forwarded to our office)
• A list of all medications you are currently taking
• Dental insurance card, if you have insurance
